Replace YOUR_ACCESS_KEY with your form's public key. The code includes its styles and an AJAX enhancement; the native HTML POST also works without JavaScript.

Install this template

  1. Place the form on your website’s contact page.
  2. Choose a verified address to receive notifications.
  3. An email field lets you reply to visitors without publishing your address.
  4. Create your form using “Use this template”. Its fields and your customizations will carry over.
  5. Verify the recipient, publish the form, then replace the key in the copied code.
  6. Send an identified test and inspect the stored message and then its delivery.

Frequently asked questions

Does the form work without PHP?

Yes. Your browser sends fields to the service; processing happens on the shared backend.

Is my address visible in the code?

The code uses a public form key without showing recipients configured in your account.
